FacebookTwitterEmailLinkedin The Transport Safety Investigation Bureau of Singapore (TSIB) has published its investigation report regarding the fatal accident of a Chief Officer onboard a bulk carrier that occurred last year. On 28 October 2025, a Singapore-registered bulk carrier was transiting the Mozambique Channel en route to Durban, South Africa, to load chromite ore when its Chief Officer was found lying on the tank top inside the No. 1 cargo hold at about 1155. Despite immediate first aid, the officer could not be resuscitated.
