FacebookTwitterEmailLinkedin The UK MAIB has issued an investigation report into an incident where a pilot fell from a ladder after likely suffering a cardiac event while boarding the roll-on/roll-off cargo vessel Finnhawk. The incident At about 1300 on 8 January 2023, a Humber pilot fell without warning from the pilot ladder while boarding the roll-on/roll-off cargo vessel Finnhawk in the approaches to the Humber Estuary, England. The pilot landed on the deckhouse and safety rail of the pilot vessel Humber Saturn, sustaining back injuries before falling overboard. He could not be recovered from the water as the pilot vessel’s man overboard recovery platform could not be raised.
