The new documents contain several updates, including; • New addendum forms for barge involvement, personnel casualties, witnesses and drug and alcohol testing; • Streamlined data fields to align with statutory and regulatory language; and • The ability for a form to be completed electronically, including the use of a digital signature. New forms and supporting documents can be obtained by going to U.S. Coast Guard Homeport and clicking ‘Investigations’ on the left-hand side of the screen. The new CG2692 forms are located under the header ‘Casualty Reporting Forms’. Additional information, and an instructional video of the new form CG-2692, can be found here.
