Change of the Format of Certificate of Entry from 2009 Policy Year 9 February 2009 No.562 We would like to advise you that the form of our Certificate of Entry shall be changed from only one page of A3 size paper (420mm x 297mm) to several pages of A4 size papers (297mm x 210mm) from the 2009 Policy Year. Please be advised that we are going to send the original Certificate of Entry on which the rate and amount of premium are stated to you by mail and also send a Certified Copy of the Certificate of Entry, the column of premium of which is stated as "AS AGREED" in electronic file to you by e-mail.
