Lessons Learnt: High-Pressure System Failure High-pressure systems are widely used throughout ships and are essential to many maintenance activities. However, when the risks associated with stored energy, equipment condition and line-of-fire hazards are not fully appreciated, the consequences can be severe. This video examines a serious injury sustained during maintenance on a main engine cylinder unit following the failure of a hydraulic tool. The case highlights the importance of equipment integrity, effective risk assessment, energy isolation and safe positioning when working with high-pressure systems. While centred on an engineering task, the lessons are relevant to all personnel involved in planning, supervising or carrying out maintenance activities on board.
